Insurance and Payment Policies
The Center for Sleep and Pulmonary Medicine accepts most common forms of insurance, including Medicare. You will be required to pay any annual deductible, co-insurance or co-payment at the time of your appointment. We will file necessary forms for your primary, secondary, and supplemental insurance. You may be responsible for 100% of charges for non-covered services and should become completely familiar with the details of your specific insurance plans. You will be asked to sign an advanced notice of beneficiary form if your plan does not cover a required service. Any requests for special financial arrangements or questions about billing matters can be directed to our patient accounts office at 239-325-2052. For your convenience, we accept cash, personal checks, Visa and MasterCard. Please be sure to bring your insurance card and preferred method of payment to each visit.
