By signing up for our patient portal, established patients will have limited access to our Electronic Health Record system. We allow participating patients to request non-urgent appointments, referrals and prescription refills for non-controlled substances, as well as to receive laboratory test results, to view their health summary (including medications, allergies, basic medical history and immunizations), and to e-mail select members of the staff with non-urgent health questions or concerns.
Please bring the completed form with you on your next visit, mail it to our Naples office, or fax it directly to us at 239-263-8592. Once your account has been activated by a member of our staff, you will receive temporary login and password information so that you can create your permanent login and password and utilize our patient portal services.
